In addition, the HCAT Clinician offers crisisintervention, community assistance and coordinates the development of adischarge plan for our at-risk patient population. The HCAT Clinician alsofunctions as a member of the Interdisciplinary Patient Care Team, whocommunicates and receives referrals from other patient care areas.What you will do in this role:You will perform a screening assessment on each assigned patient, toinclude: a review of medical records, psychological risk, protectivefactors and previous assessmentsAssess your patient via tele-healthYou are responsible for completing follow-up documentationReview incoming clinical packets for potential admission to the in-patientpsychiatric units, within 30 minutes of receiptDevelop and maintain a working knowledge of Meditech, EDM, and HorizonPatient Folders, as well as any other software incorporated into the HCATProgrYou will develop and maintain communication from arrival to completion ofassessment with patient, family, physicians, case managers, hospitalpersonnel, and additional external sources to facilitate the progress of thepatient continuum of careYou are responsible for coordinating legal processes concerning patientvoluntary and involuntary admissionsTheAccess Response Center (ARC)is a state of the art Behavioral Healthclinical assessment and patient placement facility located in the MedicalCenter of Aurora.The ARC clinical team is composed of Therapists, SocialWorkers, Counselors and Nurses who provide mental health assessments forpatients of all ages located throughout the hospitals of theContinentalDivisionwho may benefit from inpatient and outpatient psychiatric care. Thewide variety of patient types that our clinical teams assess creates a dynamicand compelling experience for its professional staff.
