Colorado Department Of Human Services FT CNA/Client Care Aide II at Fitzsimons Veterans Community in Aurora, Colorado

These positions are open to Colorado Residents only.Additional important information about this position is available on the Stateof Colorado Job Board posting.Description of JobUnder the direction of the Residential Coordinator (RC) and RegisteredNurse (RN), the purpose of this position is to provide a safe andtherapeutic milieu by providing safety checks, taking measures to maintainindividuals/staff safety, and actively participating in programming forthe assigned home(s).Position may be required to stay for multiple shifts due to inclement weatheror staffing shortageJob Duties:Reviews and follows individual's individual plans,Supervises residents during activities, ensures the nursing care (bothbehavioral and medical)Advocates for the individuals they serve, teaches self-help skills with atraining approachParticipates as a member of the multidisciplinary teamsEducates families and other community co-members, and attends in-servicesAdministers formal and informal programs within the scope of practice of thisclassificationRecords and reports behavior and observations, and refers to RN forappropriate assessment.Physically lifts, transports and/or positions individualsAssistance as needed to ensure residents' needs are met in all areas ofdaily living, eating, hygiene, dressing, bathing, toileting andclothing care, personal shoppingUnderstands all aspects of fundamental rights, including the right to befree from restraints and free of abuseReports abuse allegations, claims of harassment/workplace violenceimmediately to the Emergency On Call Personnel (EOC) and Quality Assurance.Assures confidentiality of medical record and information per HIPAAregulationsAdministers nursing care and positive supports.Familiarizes self with individual's allergies, sensitivities tofood/drug interactions.Documents outcomes of program implementation in a timely and accuratelymanner.Records a wide range of information: residents' responses to training,intake, output, menses, seizures, and all information relevant to allaspects of the client's daily living and medical status.Prepares meals to prescribed diet consistencies as specified in the dietaryplanning menu.Documents variances and reports as directed by Dietary staff.Follows and maintains infection control standards and regulations.Maintains required training and certifications.Completes, reports for: incident reports, injury on the job reports,debriefing paperwork, progress notes, etc.Actively participates in the QAPI process at the home and Day program asdirected by Leadership Team.Minimum Qualifications, Substitutions, Conditions of Employment and AppealRightsEducation:High school diploma or General Education Diploma (GED)Substitution:Current, valid certification as a Nurse Aide (CNA) from the ColoradoDepartment of Regulatory Agencies will substitute for the high school diplomaor GED."Note: When using a substitution a copy of your (CNA) certificationmust be attached to your application"ANDOne (1) year of experience working with residents with developmentaldisabilities, physical handicaps, and multiple medical diagnoses in ahospital or nursing home environment.Substitution:Current, valid licensure as a Practical Nurse (LPN) from the ColoradoDepartment of Regulatory Agencies or any state participating in the NurseLicensure Compact will substitute for the experience requirement."Note: When using a substitution a copy of your licensure as a (LPN)must be attached to your application"Special Qualifications:Required to obtain a CPR card and possess and maintain a Colorado C.N.A.certification in good standing.Preferred Qualifications:Previous experience working with clients with developmental disabilitiesFamiliarity with computer data entryVersed in "Best Practice" CNA skillsExperience following and documenting individual support planConditions of Employment:Must be willing to submit to a background check, pre-employment drug screening
