We want your knowledge and expertise!Here is some of what you can expect from being a Social Worker III with us:You will be responsible and accountable for assessing the psychological,social, emotional, cultural and/or financial needs that affect high-riskpatients' health and recoveryYou will utilize social service, supportive counseling, and clinicalknowledge in the assessment of patient needsYou will be responsible and accountable for coordination of services throughan interdisciplinary process which provides a clinical and psychosocialapproach through the continuum of care and balances individually identifiedpatients and family needs with the efficacious and cost effective utilizationof resourcesYou will participate in patient assessment to determine appropriateness ofadmission, continued hospitalization, as well as appropriate level of careYou will participate in discharge planning, which will begin at the time of(or prior to) admission, and reassessed ongoing throughout the course ofhospitalization.You will use your knowledge of payer source implications on discharge plan andaddresses assessed needs in a timely manner.You will use your knowledge of resources and placement options to maximizepatients' successful dispositionQualifications you will need for this role:Licensed Clinical Social Worker (LCSW) in the state of Colorado preferred.Specialty certification in case management preferredMaster of Social Work (MSW) from an accredited school of social workrequired. Doctorate of Social Work (DSW) from and accredited school ofsocial work preferred3-5 of year's recent clinical social work experience in an acute care setting5-7 years of recent clinical social work preferredSupervision experience in an acute care setting preferredMust have excellent written, verbal and interpersonal communication skillsMust be able to work with highly diverse populationMust be knowledgeable of State and Joint Commission standardsMust have ability to perform multiple tasks simultaneously and work in highstress environmentMust be able to apply critical thinking and formulate diagnostic impressionsbased upon review of systems, history, symptoms and current presentationMust be able to provide crisis intervention and utilize system...
