Denver Health RN Care Coordinator in Denver, Colorado
Job Summary Under general supervision, the RN Care Coordinator (RNCC) is responsible for facilitating and coordinating the care delivered to an assigned group of patients through multidisciplinary and patient/family collaboration to ensure quality and cost effective outcomes are delivered within appropriate care coordination parameters. Coordination involves assessment, planning, support, and evaluation of patient care and related outcomes. Activities to be performed are facilitation of appropriate status and level of care, screening for high risk and transition of care needs, interviews with patient/family, and progression of care facilitation with the healthcare team. Encounters occur face to face, telephonically, and electronically with patients in clinic, at home, at school, and at work. Is an active contributor in the development of systems (e.g. care maps, clinical paths) to improve the care of assigned patient populations. Helps ensure compliance with state, federal and third party payor requirements.
This position is responsible for accepting and covering intermittent shifts throughout the department; openings may be during day, evening, overnight (in Emergency Department), and/or weekend shifts.
Minimum Qualifications Education: Completion of a nursing education program that is accredited and satisfies the licensing requirements of the Colorado State Board of Nursing for Registered Nurses. BSN strongly preferred. Copies of highest appropriate education obtained needed for Joint Commission standards. Internal Applicants with an ADN will be considered.
Experience: Typically three years of direct patient care experience in acute or ambulatory care. Prefer experience in case management, care coordination, or patient navigation.
Knowledge, Skills & Abilities: Knowledge and understanding of case management/coordination of care principles, programs, and processes in either a hospital or outpatient healthcare environment. Experience with windows-based computer programs and ability to use computer for data analysis and data display required. Strong communication (written and verbal) and critical thinking skillsd. Includes the ability to receive, process and convey information in oral, written and keyboard formats; ability to read documents and assimilate ideas; and ability to work with abstract information and formulate plans of action. Professional and effective presentation skills. Ability to prepare presentations, reports or similar and present them to groups. Efficient and effective facilitation skills and negotiation. Ability to lead team members and/or staff to achieve pre-determined goals. Effectively collaborate with and respond to varied personalities in differing emotional conditions, and maintain professional composure at all times. Strong customer service orientation and aptitude required. Use nursing process to assess, identify problems, analyze and integrate data, decide on plan of action, apply appropriate intervention, and evaluate outcomes. Evidence of leadership, creativity, integrity and initiative required. Ability to integrate within team environment.
Certificate/License/Registration: Basic Life Support for Health Care Provider Certification/CPR. Active Registered Nurse (RN) License in the State of Colorado or a participating compact state. All Certifications and Licenses required for this job must be kept current as a condition of continued employment.
