Denver Health Social Worker MSW (HCM) in Denver, Colorado
Job Summary Under general professional supervision, performs social work/case management services including planning, organizing, treatment, consultation, case coordination to facilitate referrals and crisis intervention in addition to anticipating and coordinating patient discharge needs. Delivers social services in accordance with the approved plan of treatment and plan of care. Assists the provider and other team members in understanding significant social and emotional factors related to the patient. The MSW abides by guidelines and expectations specific to the clinic in which they provide patient care. Services will be provided in primary care clinics with a focus on family support and therapeutic interventions.
Minimum Qualifications Education: Master’s Degree in Social Work from a CSWE accredited college or university.
Experience: Typically, one year of experience.
Knowledge, Skills & Abilities: Knowledge of emotional and social factors that effect patient’s health and family functioning. Excellent interpersonal skills. Skilled in identifying problems/opportunities to improve care. Ability to help the patient and family to understand, accept and follow therapeutic recommendations. Ability to utilize resources, such as family and community agencies to assist the patient in the community. Ability to collect key information from the patient, family and the community as part of the social work assessment. Ability to communicate to the appropriate staff information about the patient and family’s care coordination plan. Motivation to work as part of a dynamic multidisciplinary team and participate in medical research and evaluation. Demonstrates ability to develop effective therapeutic interventions from a biopsychosocial culturally competent perspective.
Language: Fluent in Spanish highly preferred.
