Two years required experience withhomeless and/or youth populations; group facilitation and /orteaching; and crisis management. Individuals with experience in thefollowing areas will be given preference - Understanding of Trauma InformedCare; High Fidelity Wrap-Around and Critical Time Intervention models.Awareness of Denver Metro area human service resources. Computer proficiencyin Windows as well as experience with Microsoft Office Suite and databases.Own transportation with valid Colorado driver's license, up-to-dateregistration and automobile insurance. Strong organizational andcommunicationJOB RESPONSIBILITIES:The Case Manager of the Youth Transitions Project (YTP) will beresponsible for assessing the needs of each youth ages 16-24 in conjunctionwith the Bannock Youth Team. Will be responsible for the development,implementation, monitoring and evaluation of the service plans; conductingindividual and group meetings; conducting the Life Skills CompetencyWorkshops; supervising the day to day program needs of the youth.
