We want your knowledge and expertise!Here is some of what you can expect from being an RN Case Manager with us:You will coordinate services through an interdisciplinary process, whichprovides a clinical, financial, and psycho-social approach through thecontinuum of careYou will balance individually identified patient and family needs with theefficacious and cost effective utilization of resourcesYou will assess patients to determine appropriateness of admission,continued hospitalization, clinical resource consumption, progressive caremanagement, as well as appropriate level of careYou will participate in discharge planning at the time of (or prior to)admission, and reassess throughout the course of hospitalizationYou will support collaboration and communication among the patient/familyand entire health care team regarding the patient's plan of care andcontinued needs post-discharge and facilities those services requiredYou will perform telephonic clinical reviews with claims insurance personnelto facilitate reimbursement for the delivery of patient care servicesYou will identify quality and/or risk issues and reports to the appropriateindividual/departmentQualifications you will need for this role:Current licensure in the State of Colorado as a Registered Nurse, or currentactive multistate nursing licensureBLS certification through AHA or ARCGraduate of an accredited school of nursing; BSN preferred3-5 years recent clinical or case management experience in area ofspecializationExcellent human relations and oral/written communication skillsStrong analytical and organizational skills.Must have the ability to asses' information, apply critical thinkingprocesses to determine course of action, define expected outcomes, andevaluate effectiveness of plan through continuous assessment of thepatient's pMust have the ability to compile, evaluate and report statistics to membersof the team, and utilize the information to facilitate process improvementactivitiesSwedish Medical Center, part of HCA's HealthONE, is located in the southmetro Denver area where it has been a proud member of the community for morethan 110 years.
