In addition, we provide a supportiveenvironment where talents, initiative, and dedication are recognized andrewardEDUCATION/EXPERIENCE of a Case Manager: Bachelor in Social Work or relateddegree in Human Services required. Minimum 5 years experience in casemanagement or appropriate combination of education and experience required forapplicants without a Bachelor in Social Work or related degree. CaseManagement and experience working within the field of Intellectual andDevelopmental Disabilities or related systems strongly preferred.GENERAL DESCRIPTION of a Case Manager: The Case Manager is responsible forthe planning, coordination and monitoring of services to develop and attainindividual goals and outcomes with optimal participation of the person,family and extended care team. This position ensures that the health andsafety of people receiving services are met and that services are deliveredaccording to State and Waiver guidelines. Case Managers perform in-depthassessment to evaluate needs, barriers to accessing resources and servicesand works with the person and family to create and implement person-centeredservice plans.Essential Duties of a Case Manager:Assists people and families in developing goals to address areas of need andassists in developing service plans which are assessed regularly and adhere tomandated guidelines; conducts assessment and determines required level andfrequency of services.Utilizes evidence-based communication tools (i.e. motivationalinterviewing, SMART goal techniques, psycho-social assessments, statemandated assessments, etc.) to communicate with individuals andfamilies, gather information and provide person-centered approaches to goalsetting and service planning.Handles multiple case assignments and caseloads of approximately 50+,implements service plans, reviews case progress and determines case closure.Meets strict documenting guidelines and timeline for case management activityand case information. Documents accurately and timely all case managementactivity according to agency protocol and government mandated guidelines andadheres to stringent billing requirements.Facilitate multiple care aspects of a case (care team collaboration,information sharing, in-depth assessment, goal planning, follow-up,meeting facilitation, progress reviews, etc.)ESSENTIAL SKILLS of a Case Manager:Person centeredGood organizational skillsAbility to manage time wellGood communication skills able to explain concepts to people with a variety ofabilitiesAccurate and clear writing skillsAbility to use various data bases and computer programsThis position is currently
