BAYADA Home Health Care is currently seeking an experienced OccupationalTherapist, OT, for a full or part time opportunity performing home healthvisits for our Westminster, CO office.Coverage/territory: North Longmont area.As a home care Occupational Therapist, you will be an integral member of amulti-disciplinary health care team that provides rehabilitative care andskilled nursing to clients, affording them the opportunity to receive themedical care required to remain at home.Occupational Therapist, OT Responsibilities:Make home visits to clients in designated geographic territories.Perform thorough evaluations of patients' functional status, includingmuscle function, endurance, visual coordination, written and verbalcommunication skills, and ADL and IADL performance.Assess cognitive, psychological and motor aspects of performance and developindividualized intervention plans to achieve favorable outcomes.Evaluate home environment for hazards or barriers to more independent living.Identify equipment needs and environmental adaptations resulting in enhancedoutcomes for clients.Continually assess and revise the occupational therapy care plan, andparticipate with nursing in the multidisciplinary care plan, as appropriate.Educate and instruct patients, family members, or other patientrepresentatives, in rehabilitative care and activities necessary to promotethe client's health, safety and independent living.Accurately document observations, interventions, and evaluationspertaining to patient care management and services provided, utilizing astate-of-the-art touch pad tablet.Occupational Therapist, OT Qualifications:A current Colorado Occupational Therapist license.Prior home care experience preferred, but not requiredA minimum of one year of recent work experience as an Occupational Therapist.Graduation from an occupational therapy program accredited jointly by theCommittee on Allied Health Education and Accreditation of the AMA and theAOTA, as indicated by school transcript or diploma.Ability to work independently and manage time effectively.Strong interpersonal skills, organizational skills, and problem solvingskills.Solid computer skills; prior experience with electronic medical records(EMR) preferred.Additional Information:Base Pay, depending upon qualifications and subject to negotiation:Level 1: $43.20 - $46.83 per pointLevel 2: $47.32 - $51.75 per pointLevel 3: $53.93 - $60.00 per pointTo learn more about BAYADA Benefits,About BAYADAFounded in 1975 by J.
